Effectiveness of NQ-E index-based individual nutrition counseling for community-care elderly: an intervention study on improving nutritional status, complex chronic diseases, and quality of life

Objectives
This study sought to assess the effectiveness of community-based nutrition counseling on improving nutritional status, managing complex chronic diseases, and enhancing the quality of life for elderly individuals with chronic conditions, particularly in older adults with high levels of food insecurity and multiple chronic illnesses.
Methods
Thirty elderly subjects with diabetes and hypertension who were registered at local Senior Welfare Center received individualized nutrition counseling, based on their Nutrition Quotient for the Elderly (NQ-E) index. Over a 16-week period, they received tailored counseling and underwent various health and nutritional assessments. The final analysis included 28 participants after two dropped out. Data analysis was conducted using the SPSS v28.0.
Results
The subjects were over 70, with multiple chronic diseases including diabetes and hypertension and predominantly female. After 16 weeks, significant improvements were observed in the subjects’ grip strength, and HbA1c levels, as well as in their NQ-E scores, indicating improved dietary balance and diversity. There were no significant improvements in the ‘Moderation’ subdomain of the NQ-E index, suggesting that this aspect requires further attention in nutritional counseling. The subjects' nutritional risk scores (NSI) were also significantly decreased, indicating less nutritional risk. Lastly, as measured by the SF-36K, the subjects’ quality of life showed significant improvement in several domains including physical role performance and social function.
Conclusions
This study demonstrates that tailored nutrition counseling, based on the NQ-E index, can improve elderly health, manage chronic diseases, and enhance quality of life. This approach potentially broadens the scope of community nutritionists' roles within an aging society. However, additional research is necessary to evaluate these interventions' long-term effects and sustainability.

Development of 3D Printed Snack-dish for the Elderly with Dementia

Objectives
This study was conducted to create a 3D printable snack dish model for the elderly with low food or fluid intake along with barriers towards eating. Methods: The decision was made by the hybrid-brainstorming method for creating the 3D model. Experts were assigned based on their professional areas such as clinical nutrition, food hygiene and chemical safety for the creation process. After serial feedback processes, the grape shape was suggested as the final model. After various concept sketching and making clay models, 3D-printing technology was applied to produce a prototype. Results: 3D design modeling process was conducted by SolidWorks program. After considering Dietary reference intakes for Koreans (KDRIs) and other survey data, appropriate supplementary water serving volume was decided as 285 mL which meets 30% of Adequate intake. To consider printing output conditions, this model has six grapes in one bunch with a safety lid. The FDM printer and PLA filaments were used for food hygiene and safety. To stimulate cognitive functions and interests of eating, numbers one to six was engraved on the lid of the final 3D model. Conclusions: The newly-developed 3D model was designed to increase intakes of nutrients and water in the elderly with dementia during snack time. Since dementia patients often forget to eat, engraving numbers on the grapes was conducted to stimulate cognitive function related to the swallowing and chewing process. We suggest that investigations on the types of foods or fluids are needed in the developed 3D model snack dish for future studies.

Analysis of the Participation Reasons and Deterrents on Welfare Facility Dietitians for the Elderly

OBJECTIVES
This study analyzed the education participation reasons and deterrents of dietitians who work in welfare facilities for the elderly.
METHODS
The survey was completed by 144 dietitians working at welfare facilities for the elderly in Korea. The survey was conducted in October, 2018, both on-line and off-line, based on the demographic characteristics, work status on welfare facilities for the elderly, Participation Reasons Scale (PRS) and Deterrents to Participation Scale (DPS-G). The data were analyzed using frequency analysis, descriptive analysis, factor analysis, reliability analysis, regressive analysis using SPSS ver. 25.0.
RESULTS
The reason for participation were divided into three factors: ‘Responsibility of professional and self-development (5.76 ± 1.04)’, ‘Job stability and personal benefits (4.98 ± 1.28)’, and ‘Interaction and development of professional competencies (5.85 ± 1.00)’. ‘Interaction and development of professional competencies’ was the highest motivation factor. Also, the deterrents for participation were divided into four factors: ‘Dispositional barrier (2.70 ± 1.29)’, ‘Dissatisfaction of education usability (3.39 ± 1.38)’, ‘Institutional barrier (4.21 ± 1.45)’, and ‘Situational barrier (2.36 ± 1.30)’. ‘Institutional barrier’ showed the highest deterrents factor. In addition, ‘Responsibility of professional and self-development’ and ‘Interaction and development of professional competencies’ were negative attributes for ‘Dispositional barrier’ (p<0.001).
CONCLUSIONS
These results provide basic data to promote participation in education and contribute to the improvement of their job ability and education capacity of the food and nutrition management of welfare facilities for the elderly

The Factors Influencing Health-Related Quality of Life in the Elderly: Focused on the General Characteristics, Health Habits, Mental Health, Chronic Diseases, and Nutrient Intake Status: Data from the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010~2012

OBJECTIVES
This study intended to determine significant factors that influence the health-related quality of life ("HRQoL"; EuroQol 5 Dimension health-related quality of life (EQ_5D) & EuroQol visual analogue scale (EQ_VAS)) of the elderly in Korea.
METHODS
This study was based on 3,903 subjects aged 65 years or more who participated in the Fifth Korea National Health and Nutrition Examination Survey (KNHANES V), 2010~2012. The HRQoL was analyzed by various factors (general characteristics, health habits, mental health, chronic diseases, nutrient intakes). SPSS statistics for complex samples (Windows ver. 21.0) was used.
RESULTS
The HRQoL was higher in the males, those with higher educational level or higher income level while it was lower in those belong to single households. In particular, the EQ_5D was significantly higher in the group who reported walking practice, moderate physical activity (male), and the group who reported no vigorous physical activity (female). The EQ_VAS was significantly higher in the group who reported walking practice. Both EQ_5D and EQ_VAS were significantly lower in the group with stress, melancholy, suicidal thinking, and osteoarthritis. EQ_5D was significantly lower in the group with < 75% Estimated Energy Requirements (EER) in energy intake, and with < Estimated Average Requirements (EAR) in iron or niacin intake. A stepwise regression analysis revealed that i) higher educational level (male), and good self-rated health status significantly increased the EQ_5D, ii) age, alcohol intake (male), melancholy (female), suicidal thinking, osteoarthritis, and niacin intake deficiency (male) significantly decreased the EQ_5D, iii) higher income level (male) and good self-rated health status significantly increased the EQ_VAS, and iv) age (male), stress, suicidal thinking (female) and osteoarthritis significantly decreased the EQ_VAS.
CONCLUSIONS
This study suggested that general characteristics, mental health, osteoarthritis, and niacin intake were associated with the HRQoL. Prospective research of long-term control is needed to establish the causal relationship between factors and the HRQoL.

Survey on Present Use and Future Demand for the Convenience Food in the Elderly Group

This study is to research the use and the demand of the elderly generation that pursue economic ability and active lifestyle. The composition of the meals were "homemade meals" 65.3%, "homemade & convenience meals" 26.7% overall. For the "never miss a meal" category, 70~74 years old was 48.3%, over 80 years old was 60.0%. The elderly under 70 years old showed a high frequency in the use of alternative meals (p < 0.05) and the elderly with higher education used the alternative meals more frequently (p < 0.01). The rice-cakes, bread, tuna cans, Korean side-dishes, beverages, meat, cup-noodles and seafood as alternative meals are in the order of most frequency. Also, 96.0%, 59.0% and 46.5% of the elderly had experience in using "Ready to eat (RTE)", "Ready to heat (RTH)" and "Ready to cook (RTC)". The oldest group surveyed, the elderly at the age of 80 years old and above showed lower preference for "completely cooked state" but showed highest results in the "half-cooked state". This shows the difference between people's perception and the behavior that showed high frequency of 96.0% in the experience with "RTE". This can be inferred that the elderly needs the development of convenience food that is moderately packaged according to the newer lifestyle where the elderly live independent from their children. Moreover, there is a higher demand in the healthy convenience food, which means that there is a need for development in the quality of the convenience food that will allow more faith in convenience food by the elderly.

Anthropometric and Biochemical Indicators and Related Factors for the Community Dwelling Elderly Living Alone

In this study, anthropometric and biochemical indicators and related factors for community dwelling elderly living alone (ELA). as well as for elderly not living alone (ENLA) were explored. The subjects were 140 elderly females (70 living alone, 70 not living alone) residing in Bucheon. Anthropometric measurements revealed significant differences between the two groups (ELA/ENLA) for height, weight, BMI, triceps skinfold thickness, suprailiac skinfold thickness, subscapular skinfold thickness, and body fat percentage. The ELA showed significantly lower data for most of the anthropometric indices, except PIBW. Biochemical indicators of iron status (hemoglobin, serum iron, transferrin saturation) were significantly lower for the ELA, whereas the total serum cholesterol, LDL-C, LDL-C/HDL-C, for the ELA were significantly higher. The prevalence of hypercholesterolemia (240 mg%) was 30%. The latter three are not in accordance with general malnutrition among the ELA. The hemoglobin of the ELA was positively correlated with energy, fat, iron and cholesterol intake, but negatively correlated with height. The serum cholesterol of the ELA was positively associated with most of their nutrient intake.

The Dietary Behaviors, Depression Rates and Nutrient Intakes of the Elderly Females Living Alone

In this study, the dietary behaviors, depression rates and nutrient intakes were assessed for elderly females living alone (ELA) and elderly females not living alone (ENLA). The subjects were 140 elderly females (living alone 70; not living alone 70) residing in Bucheon city. Dietary data were obtained using the 24-hr recall method. There were more subjects with low monthly incomes (less than 500,000 won) in the ELA group. The proportion of the ELA group which skipped at least one meal per day was 38.5% and the main reasons given were low appetite and depression. Fewer of the ELA group were observed to have smoking and drinking habits. However, the majority of the ELA smokers smoked more than 6 cigarettes per smoking. The drinking score of the ELA group was also higher than that of the ENLA group. The total score of depression for the ELA group was higher than that of the ENLA group. The total score of nutrition risk index (NRI) of the ELA group (8.09) was also higher than that of the ENLA group (2.31). The dietary assessment using the 24 hr-recall method showed that the ELA group had lower nutrients intakes, and significant differences were shown in the intakes of energy (1137 kcal vs 1275 kcal), animal protein, animal fat, carbohydrates, animal Ca, and animal Fe. There was a positive correlation between the NRI and the depression scores. However a higher NRI was associated with lower intakes for most of the nutrients in the ELA group.

The Effect of Kimchi Intake on Free Radical Production and the Inhibition of Oxidation in Young Adults and the Elderly People

This study was performed to investigate the effect of kimchi intake on free radical and oxidative substance production in young adults and the elderly. Daily kimchi intake by people in their twenties (n = 93, 20 to 29 years old) and over sixty-five (n = 143, over 65 years old) in M city were surveyed and blood was drawn to analyze the free radicals in their plasma. The average amount of kimchi intake by the subjects was 115.8 +/- 91.7 g. The amount of kimchi intake of those in their twenties (106.1 +/- 80.6 g) was significantly lower than that of those over sixty-five (125.5 +/- 102.9 g, p<0.05). Concentrations of total free radicals and OH radicals were 27 and 33% greater respectively, in those over sixty-five than in those in their twenties, indicating that more free radicals were produced by the older group. The concentration of GSH was not signiacantly different in the two groups, but that of GSSG in the over sixty-five age group was 53% greater than in the twenties group, which resulted in a 35% reduction in GSH/GSSG in the elderly group. TBARS concentration in the over sixty-five group was 26% greater than that of the twenties group. In order to see the effect of kimchi intake on free radical production, subjects in same age group were divided into two sub groups-the mean over and the mean under groups-according to the average amount of kimchi intake, which was 115.8 g. The total free radicals, the OH radicals, the GSH, the GSSG, and the GSH/GSSG in the twenties group were not significantly different in the two kimchi intake groups. However, those in the over sixty-five group were significantly different. The concentration of total free radicals and OH radicals of the mean over group were 21 and 26% lower respectively, than those of the mean ova. group (p<0.05). The GSH and GSH/GSSG of the mean ova. groups were higher by 8 and 12%, respectively. The correlation coefficient between the kimchi intake and the total free radicals was -0.1862 (p<0.05) and that for GSH/GSSG was 0.1861 (p<0.05). In conclusion, the production of free radicals and oxidative substances increased with age, and kimchi seemed to retard this phenomena.

Physical Characteristics and Instrumental Activities of Daily Living of the Elderly(85+) in Kyungpook Sung-Ju Area

The purpose of this study was to investigate the physical characteristics and instrumental activities of daily living(IADL) of the elderly (85+). The subjects were three-hundred elderly people over the age of 85, living in Kyungpook Sung-Ju area who have no problems in daily living. The survey on anthropometric measures and IADL were carried out by individual measurement and interviews using measuring devices and questionnaires. The subject group of the study was composed of 27% males and 73% females, the average age being 87 years old. Notable physical features of the subjects include considerably short statues and small physiques compared to the average Korean elder. The mean Body Mass Index(BMI) was 20.9. The average percentage of body fat in male and female was 21.7% and 29.5% respectively, and the average waist/hip ratio(WHR) was 0.9 and 0.85 respectively. The percentage of the subjects who were able to perform all the IADL was 51% in males and 25% in females. The male subjects were more capable than the females in all the IADL except for preparing meals. BMI showed positive correlations withy weight(r=699), WHR(r=0.157), body fat percentage(r=0.57), lean body mass(r=0368) and total body water(r=0.368). WHR also had positive correlation with weight(r=0.184), BMI(r=0.157), lean body mass(r=0.149) and total body water (r=0.148). Body fat percentage had a positive correlation with weight(r=0.147) and BMI(r=0.265) and total body water(r=0.265). In summary, the elderly(85+) in the Sung-Ju area had considerably smaller physiques than the average Korean elderly and showed a normal range of BMI in average. The IADL of the elderly(85+) appeared to have a positive correlation with lean body mass and total body water and a negative correlation with body fat percentage.
